Pain and Performance Issues of theses extremities usually can be attributed to three things.
A direct injury. Example falling, car accident, work injuries, blunt trauma, or sports accidents(jpegs)
Direct ongoing micro traumas or ongoing overuse injuries. Example Computers, Manual Labor, Sports, or the incomplete rehabilitation of an injury. (jpegs)
Indirect micro traumas that occur from the excess stress placed on a joint due to the instability of the muscle, spine and pelvis. Example Loss of curvature of the spine leads to excessive load on the body and can shorten or lengthen various muscles and connective tissue altering their ability to operate in an efficient manner.
Example:
A patient presents with anterior(front) right knee pain. The patient is 46 year old male who is an avid runner and reports a previous injury to his left ankle approximately five years ago. He said he was running on his street and stepped of the curb to cross to the other side when he rolled his ankle. He has been an accountant for the last 23 years and exercises two times per week with weights and runs either on the treadmill or outside three times per week at 3 miles per run. He is at his ideal weight and eats healthy. The pain has been on and off for the last five months and recently it has gotten worse and reported some swelling to the area.
Ok, so as the doctor it would be easy to exam the knee send for diagnostics such as X-Ray or an MRI and see if there is tissue damage or bone issues. We may find some micro tares but most likely the results may be inclusive because there was no direct injury. If we find issues we may decide surgery is necessary.
But what if we took a whole other approach to this issue and look for the cause of symptoms. This is a 46 year old male who has worked hard at staying healthy. Doing surgery and taking away his healthy lifestyle could be very harmful to this patients way of life.
So we first look at the previous injury and find that it was never properly rehabilitated and although the pain subsided the ankle had lost it's structural integrity.
So how do we treat these injuries?
We start with a movement analysis of all functional movements of the body.
We take necessary diagnostics such as X-Rays, MRI or Ultrasound.
A combination of joint mobility techniques, muscle therapy and pain reducing modalities such as kinesio tape, electric stimulation etc... Once we have restored motion and reduced the pain we look to strengthen the are with a formula of stretches and exercises that will be added to your current exercise program.
